Placement & Brokerage Officer needed in Birkenhead Paying  £14.28 per hr ref OR110510

Full time hours on a temporary basis

• Key ResponsibilitiesCare Package Brokerage and Coordination
  • Arrange and record care packages accurately using designated case management systems, ensuring compliance with organisational procedures and quality standards.
  • Source and broker care and support services from contracted providers in line with assessed needs, care plans, and available resources.
  • Manage daily referrals and demand for care services, prioritising urgent cases including hospital discharges and safeguarding situations.
  • Support the coordination of out-of-area placements, ensuring continuity of care and adherence to relevant policies and procedures.
  • Ensure care arrangements are implemented within required timescales to support effective service delivery.
