Outreach Worker needed in North London Paying  £17.40 per hr ref 013266

Full time hours on a temporary basis

Key Responsibilities

• Act as a single point of contact for identified rough sleepers, supporting their transition into emergency or temporary accommodation.
• Collaborate with the Rough Sleeper Co-ordinator and Single Homeless Officer to develop tailored action plans.
• Ensure individuals engage with relevant support services to sustain tenancies and prevent repeat homelessness.
• Work in partnership with housing, health, and social care teams to deliver a multi-agency approach.
• Maintain accurate records and contribute to strategic planning and reporting.
