Placement & Brokerage Officer needed in Birkenhead Paying  £12.81 per hr ref OR105212

Full time hours on a temporary basis  

Key Responsibilities

• Coordinate and arrange care packages and placements for adults in line with assessed needs and care plans
• Support safe and timely hospital discharges, reducing delays and improving outcomes
• Liaise with care providers, agencies, and internal teams to secure suitable services
• Source and negotiate placements that provide value for money while maintaining quality
• Maintain up-to-date records and ensure compliance with policies, procedures, and legislation
• Monitor availability and capacity within the local care market
• Build and maintain strong working relationships with providers and stakeholders
