Delayed Transfer Of Care Screening Officer (Hybrid) needed in Morden, £20.35ph PAYE – Reference: 5293670

  • To be responsible for the identification, monitoring and recording of delayed transfers of care relating to Merton residents across a range of hospital sites, to keep Delayed Transfers of Care (DTOC) to a minimum and ensuring accurate auditable data is shared with Merton CCG and NHS England.
  • Responsible for screening and assessing every referral and deciding the most appropriate pathway for each individual. Consideration must be given to the information provided, the complexities of each case and the profession (s) or sector (s) best placed to take the lead.
  • Undertake robust communications with the acute trusts to ensure discharges from hospital are safe, readily equipped and appropriately arranged within the time frame set out in Government’s guidelines and the local protocols.
  • Undertake complex investigations with both health and social care teams to identify the facts behind any delayed discharges.

2.1) To develop and maintain expert knowledge of care pathways in order    to determine the appropriate pathway (or pathways for very complex cases) for each referral.

2.2) Recording the referrals through to the new Mosaic recording process (new Adult Contact, D2A work step) as such, including safeguarding and then alert Duty Manager for complex cases such as cases for allocation and cases for safeguarding; clarity of pathways; potential referrals rejections etc.

2.3) Ensure that D2A work step is completed within 24 hours of starting the work step- referrals are either accepted or rejected.

2.4) Referrals for the Hospital Team to be located into the Team’s waiting list virtual residents and not on the Hospital to Home Current work to meet CQC requirements.

2.5) To forge links with the Occupational Therapy Department, Community Social Work Teams, Hospital Social Work Teams and Brokerage as required to acquire expert knowledge of the service specifications and interfacing processes.

2.6) Complete Reablement referrals and liaise with Reablement and hospitals with regards to start of package of care, confirming equipment, arranging key safe services via the handyman.

2.7) To lead on ensuring validation of information recorded by Acute Hospital Trusts in accordance with the protocols, contesting any reimbursement fines against the council accordingly.

2.8) Provide daily updates for St Helier and other hospitals.

2.9) Complete mascot referral for telecare installation.

 

This is a full time role on a temporary contract basis.
