Adult Social Care Placement Coordinators (Hybrid)

Adult Social Care Placement Coordinators (Hybrid) needed in Southampton, £14.92ph PAYE – 5305227

1. To receive referrals from practitioners and secure appropriate support packages/placements.
2. To have access to the social care records of vulnerable adults.  To also have direct contact where appropriate with vulnerable adults and/or their representatives to advise on details of support secured for that individual and or/provide guidance on the actions the Placements Service will take to secure that support.
3. To accurately create individual contracts, to enable payments to be made to providers within agreed timescales. These may include one-off arrangements, or amendments to existing arrangements.
4. To ensure effective communication with practitioners, budget holders, providers, commissioners and finance teams and to carry out speedy resolution of queries.
5. To inform manager about any gaps or trends identified in the market place when sourcing support packages or Placements.
6. To monitor occupancy and maintain a register of internal and external services to inform sourcing of Placements. 
7. To assist management with information reports relating to the duties of the post.
8. To undertake other duties commensurate with the grade, as required.
