Neighbourhood Model Program Lead Full Time Temporary

Neighbourhood Model Program Lead needed in Barnet paying £288.65 per day – Reference: RQ657445

This is a full time role on a temporary basis

 This role will involve:

• The post holder will work closely with CCG Directors, the wider local leadership team and the Head of Partnership to take forward the development of the Integrated Care Partnership and ensuring its aims and objectives are successfully delivered.
• The post holder will lead the design, planning and implementation of the neighbourhood model programme for the Partnership as an initial priority, and other programmes of work as the role and priorities evolve over time.
• Support NCL CCG and its partners to deliver a meaningful reduction in health inequalities and a system shift towards person centred care, prevention, early intervention, self-care and self-management. Works with commissioners and providers to ensure the processes and incentives in the system, support integration not fragmentation or silo working (for example, approaches to planning, service (re)design, business case development, payment models, contracting models, performance frameworks etc.)
